sketch your form with paper and pencil, this will give you a guide to work with use your existing contact.html file or create a new document from your subpage template select and delete any text, except the heading, you may have between the "contentAboutText" div tags
the contentAboutText box should now be blank (except for the heading)... it should look something like this:
